Filling in the blanks is the easy half. It still has to be executed on stamp paper of the value currently prescribed, signed by the right people in the right order, and — depending on the document — sworn before a Notary or registered with the Sub-Registrar. A correctly worded document on the wrong stamp, or signed by the wrong person, fails just as completely as a badly worded one.
Bring it in and we will check it, e-stamp it and notarise it in the same visit. If it is wrong for what you actually need, we will tell you before you pay for anything.
Report it first, then swear it
Most issuing authorities want two things: proof that you reported the loss, and an affidavit that you lost it. Do them in that order. Lodge a police complaint or obtain a non-traceable certificate first, then swear the affidavit referring to that complaint by number and date. An affidavit that does not mention a police complaint is frequently sent back where the document has any financial value — share certificates, fixed deposit receipts and vehicle registration certificates in particular.
Under the Ministry of External Affairs' simplification of the passport rules, every annexure is now a self-declaration on plain paper, and no attestation or swearing before a notary, executive magistrate or judicial magistrate is required. That includes Annexure F for a lost or damaged passport — despite its heading and its "solemnly affirm" wording, the current specimen carries no jurat at all. Some Passport Seva Kendras still ask for notarisation in practice, and if yours does we will do it. But do not assume it, and do not pay for notarisation of a document the rules do not require. Use the passport office's own annexure rather than a general format.
Describe the document precisely
"I lost my certificate" is not enough. The affidavit should carry the document's number, the date it was issued, the authority that issued it, and the name it was issued in. If you do not have the number, say so explicitly rather than leaving the field blank — a blank reads as an incomplete affidavit, whereas a stated "the number is not known to me" is a fact you can swear to.
What to bring when you come in
- Police complaint copy or non-traceable certificate, where you have one
- Any photocopy, scan or photograph of the lost document
- Aadhaar or other photo identity
- The issuing authority's written requirement, if they gave you one
