Filling in the blanks is the easy half. It still has to be executed on stamp paper of the value currently prescribed, signed by the right people in the right order, and — depending on the document — sworn before a Notary or registered with the Sub-Registrar. A correctly worded document on the wrong stamp, or signed by the wrong person, fails just as completely as a badly worded one.
Bring it in and we will check it, e-stamp it and notarise it in the same visit. If it is wrong for what you actually need, we will tell you before you pay for anything.
This is not a name change affidavit
The distinction matters and people get sent back for getting it wrong. A change of name affidavit says: I was known as A, I now wish to be known as B. A one and same person affidavit says: I have always been one person, some records say A and some say B, and both are me. Nothing is changing. Banks, PF offices, insurers, universities and the passport office ask for this one far more often than they ask for a change of name.
Use it where the difference is a spelling variation, an initial expanded or contracted, a surname order swap, or a middle name present in one record and absent in another — Ramesh K. Patel against Rameshbhai Kanubhai Patel, for example. If you are genuinely adopting a different name, use the change of name affidavit instead.
List every variant
The single most common failure is listing two variants when the file contains three. Before you swear it, lay every document side by side — Aadhaar, PAN, bank passbook, school leaving certificate, property papers — and write down exactly how the name is printed on each one, character for character. Each variant goes into the affidavit against the document it appears on. An affidavit that covers two of three spellings is very likely to be sent back by the authority holding the third.
What to bring when you come in
- Every document that carries a different spelling — bring the originals if you can
- Aadhaar or other photo identity
- A note of which authority is asking for the affidavit, and what they asked for in writing
